To help streamline your check-in process at your first appointment, we encourage you to download, print, and complete these forms in advance. Although not required, bringing the completed forms with you will save time and will ensure a smoother visit.

What do I need to bring to my first appointment?
For your first appointment at Beaver Medical Group, please bring the following:
- Medications or a Medication List
It’s best to bring the actual medication bottles so your provider can review the exact dosages and instructions. - Photo ID
A valid government-issued identification is required. - Insurance Card
Bring your current insurance card for verification. - Forms for First Appointment
While these forms will be available at the office, you can DOWNLOAD THEM HERE and complete them in advance to save time. The required forms include the New Patient form and the Credit Card on File form. - Arrival Time
Please arrive 30 minutes early to the designated appointment time. This allows enough time for completion of forms and updates.

What happens if I no show to an appointment?
Patients who do not show up for their appointment will be charged with a $25 no show fee. Patients should cancel 24 hours in advance of their appointment for no penalty.
What happens if I arrive to my appointment late?
You may be subject to reschedule appointment depending on availability of provider at time of arrival.
